If you use the contact form, your name, phone number, email and message are sent to the practice so we can respond. Please do not include health information in the form; clinical matters are discussed by phone or in session.

Clinical records
Once you become a client, your records are protected under applicable California and federal law, and you'll receive a separate Notice of Privacy Practices.
